IMPORTANT:

• Pre-registration is required! This will allow Dr. Oliver to have you and your horse’s information already entered into her database and allow her to have enough vaccinations to cover everyone’s needs. Everyone will also be given a time slot to streamline the process. Fill out the form below to pre-register. Or, you can e-mail or call Dr. Oliver with your information . We will need you to pre-register by March 25st, 2022.
  
• Information required:
  • Your name, address, phone number and e-mail address (for Coggins test)
  • Your horse’s name, age, breed, color and gender
  • The requested vaccinations for your horse(s) and/or if you would like a Coggins
  • Vaccine history and any known vaccine reactions.
• Payment will be required at the time of service. CEVS accepts cash, checks and the major credit cards.
• Please do not bring sick horses to the clinic. Any horses with a cough, fever or other signs of sickness should be left at home.
• Feel free to call Dr. Oliver if you need to discuss what vaccines are advised for your horse(s). Horse’s being vaccinated with any vaccine for the first time will need boosters.
• There is NO farm call charge NOR a vaccine exam fee.
• Vaccines will not be sold for horses not at the clinic.
• Decisions on weather will be made on Friday, April 8th. You will be contacted if the date has to be moved. We will only move the date for more severe weather like extensive rain, snow, sleet, etc. We will not re-schedule if it is just cold and/or windy.

*Strangles vaccination will not be performed at this clinic. Dr. Oliver believes administering this vaccine should be done on a separate day from other vaccinations due to the potential for side effects. Please let Dr. Oliver know if you would like this vaccine. If there is a large need for this vaccine then it is possible another vaccine clinic can be organized.
